Customers with alternate billing and shipping addresses:
If you want your order shipped to a location other than the confirmed billing address of your credit card, you must download our Alternate Addresss Form (PDF or MS Word ) and return a 100% completed copy via email or fax before calling. JSC Speed, Inc. will only ship to an alternate address if this form is completed, we will not ship to a non-confirmed address with out this form being 100% complete. This form is only valid for US residents and only for orders placed using a credit card. The Alternate Address Form is not valid for PayPal orders or customers with billing address outside of the the United States.
